All CHCW patients are offered patient-centered, on demand, compassionate focused, integrated behavioral health services. Our model of care follows the GATHER acronym (Generalist, Accessible, Team-oriented, High productivity, educator, routine) and strives to help our medical system reach the 4 C's of primary care (first contact, continuity of care, comprehensive care, and coordination of care).
