PCBH is an integrated service model for primary care settings, where behavioral health services are offered as part of routine medical care. PCBH providers conduct universal behavioral health screenings, provide problem-focused short term interventions, and communicate/collaborate with the PCP to identify any additional behavioral health services, if needed.
